Our Social Workers are advocates for you and your loved one. We value your uniqueness and self-determination. No judgment is made about a person's preference, choice, gender, age, or economic status. Assistance from our Social Workers is free of charge.
Deer River HealthCare Center (DRHC) employs two experienced Social Workers. Both maintain a current license with the Minnesota Board of Social Work. We continually acquire skills and techniques to maintain and enhance your continuity of care. We provide medically related social services to attain the highest practicable physical, mental and psycho-social well being of each assigned resident/patient of the Deer River HealthCare Center.
Enhancing continuity of care
- Coordinate admissions and discharges for the Deer River HealthCare Center Homestead facility and Community Memorial Hospital. Our Social Workers will work with you to establish a plan for a safe transition from our facility to your home or any other placement you choose. We can assist you with referrals to various support services in your community. If you choose discharge from the hospital into Homestead short or long term care, our Social Workers will follow you and continue to address your needs.
- Conduct ongoing assessment and planning of individual and group needs.
- Advance Medical Directive/Power of Attorney (POA) for Health Care or Property can be completed within our facility. Our Social Workers will support your right to healthcare decisions. Visit the Social Service office for copies and step by step assistance with completion of these documents.
- Medical Assistance applications can be obtained in the Social Service office. Our Social Workers can assist you in completing the necessary forms to obtain financial assistance.
- Consultation to alleviate stressors related to illness, life changes or admission to a skilled nursing facility.
